Printable check request - print and fill out this form and attach your receipts to the top right corner. It can be turned into the blue PTA folder in the main office or you can mail it to the school. (please include your address if you would like to have your check mailed to you and write "mail it to me" on the form also otherwise you will be notified when your check is available to pick up in the main office)
